RESOLUTION AUTHORIZING A SUB-RECIPIENT GRANT AGREEMENT WITH HEATHLY ALLIANCES MATTER FOR ALL EXTENDING THEIR COVID-19 CRISIS RESPONSE PERIOD OF PERFORMANCE TO JUNE 30, 2023.

CITY PROPOSAL:

RESOLVED, that the proper city officials are hereby authorized to execute a sub-recipient grant agreement, substantially the same as the attached Exhibit A, with Healthy Alliance Matter for All, to extend their COVID-19 crisis response period of performance to June 30, 2023 to hire at least three dislocated workers enrolled with Workforce Development as community health workers to assist with COVID-19 crisis response; the total amount to be paid by under this agreement includes only the remaining unspent funds from the Grantee’s original agreement #24157, and the total of both the original agreement #24157 and this agreement shall not exceed $58,336 payable from fund No. 268-031-6230-WD30COV; the period of performance for this agreement will be July 1, 2022 through June 30, 2023.

 

Statement of Purpose

STATEMENT OF PURPOSE:  The city’s workforce development department received an extension to the end date to June 30, 2023 for grant ID 1040100, a grant in the amount of $220,000 from the State of Minnesota, acting by and through its Department of Employment and Economic Development, Employment and Training Division ("DEED") under the COVID-19 Disaster Recovery Dislocated Worker Grant Program.  This resolution authorizes city officials to execute a sub-recipient grant agreement with Health Alliances Matter for All to extend their COVID-19 crisis response period of performance to June 30, 2023. The total amount to be paid under this Agreement includes only the remaining unspent funds from the Grantee’s original agreement #24157, and the total of both the original agreement #24157 and this agreement shall not exceed $58,336.  Funds will be used to hire at least three dislocated workers as community health workers to assist with COVID-19 response, including building partnerships to bring testing and vaccine clinics to underserved communities, connecting African Heritage and Communities of Focus to vaccines, connecting testing and vaccine participants to community resources and handing out PPE. The period of performance for this agreement will be July 1, 2022 through June 30, 2023.
