RESOLUTION AUTHORIZING A SUB-RECIPIENT GRANT AGREEMENT WITH HEATHLY ALLIANCES MATTER FOR ALL EXTENDING THEIR COVID-19 CRISIS RESPONSE PERIOD OF PERFORMANCE TO JUNE 30, 2023.

CITY PROPOSAL:
RESOLVED, that the proper city officials are hereby authorized to execute a sub-recipient grant agreement, substantially the same as the attached Exhibit A, with Healthy Alliance Matter for All, to extend their COVID-19 crisis response period of performance to June 30, 2023 to hire at least three dislocated workers enrolled with Workforce Development as community health workers to assist with COVID-19 crisis response; the total amount to be paid by under this agreement includes only the remaining unspent funds from the Grantee's original agreement #24157, and the total of both the original agreement #24157 and this agreement shall not exceed $58,336 payable from fund No. 268-031-6230-WD30COV; the period of performance for this agreement will be July 1, 2022 through June 30, 2023.

Statement of Purpose
STATEMENT OF PURPOSE: The city's workforce development department received an extension to the end date to June 30, 2023 for grant ID 1040100, a grant in the amount of $220,000 from the State of Minnesota, acting by and through its Department of Employment and Economic Development, Employment and Training Division ("DEED") under the COVID-19 Disaster Recovery Dislocated Worker Grant Program. This resolution authorizes city officials to execute a sub-recipient grant agreement with Health Alliances Matter for All to extend their COVID-19 crisis response period of performance to June 30, 2023. The total amount to be paid under this Agreement includes only the remaining unspent funds from the Grantee's original agreement #24157, and the total of both the original agreement #24157 and this agreement shall not exceed $58,336.
