RESOLUTION AUTHORIZING THE EXECUTION OF 2016-2018 PROFESSIONAL SERVICES AGREEMENT WITH THE DULUTH JOINT POWERS ENTERPRISE TRUST FOR CITY TO PROVIDE WELLNESS COORDINATOR SERVICES AND ACCEPTING REIMBURSEMENT FOR SERVICES IN AN AMOUNT NOT TO EXCEED $70,000 PER YEAR.

CITY PROPOSAL:
RESOLVED, that the proper city officials are hereby authorized to execute the agreement for wellness coordinator professional services, substantially in the form of that attached as Exhibit A, with the Duluth Joint Powers Enterprise Trust for the for the provision of wellness coordinator professional services to the Duluth Joint Powers Enterprise Trust for the years 2016-2017 in an amount not to exceed $70,000 per year; said funds to be deposited as follows: Fund 110-121-1106-4654 (General Fund, Public Administration, Human Resources, Other Reimbursements).
Statement of Purpose
STATEMENT OF PURPOSE: The Duluth Joint Powers Enterprise Trust is a joint powers enterprise established pursuant to Minn. Stat. §471.59 consisting of the City of Duluth, the Duluth Airport Authority, the Duluth Entertainment and Convention Center, and the Duluth Housing and Redevelopment Authority. The Trust operates a joint-self medical and dental insurance pool for eligible employees, retirees, and their dependents. This resolution extends the 2015 professional services agreement with the Duluth Joint Powers Enterprise Trust through which the City’s Wellness Coordinator provides wellness coordinator services to the Duluth Joint Powers Enterprise Trust. The City-paid Wellness Coordinator salary and fringe benefits are wholly reimbursed by the Trust pursuant to this agreement.
