RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE SERVICE AGREEMENT WITH THE ARROWHEAD HEALTH ALLIANCE EXTENDING THE TERM OF THE AGREEMENT THROUGH JUNE 30, 2019.

CITY PROPOSAL:

RESOLVED, that the proper city officials are hereby authorized to execute an amendment to the service agreement with the Arrowhead Health Alliance (AHA), substantially the same as the attached Exhibit A, to extend the term through June 30, 2019 for the engagement of an independent contractor to serve as a part-time coordinator for the Head of the Lakes Collaborative Crisis Intervention Team.

FURTHER RESOLVED, that the proper city officials are hereby authorized to accept reimbursement monies from the AHA, with reimbursement monies to be deposited in fund number 215-200-2220-4654 (Duluth Police Grant Programs, Police, Arrowhead Health Alliance, Other Reimbursements).                                            

Statement of Purpose

STATEMENT OF PURPOSE:  This resolution authorizes the proper city officials to execute an amendment to the service agreement with the Arrowhead Health Alliance (AHA), extending the term of the agreement through June 30, 2019.  This will enable the Duluth Police Department to continue for an additional month the services of the independent contractor currently serving as the part-time coordinator for the Head of the Lakes Collaborative Crisis Intervention Team (CIT).  This amendment will also enable the Police Department to receive reimbursement monies from the AHA for coordinator services provided through the extension period.  The Coordinator’s role is to develop, organize and standardize trainings, enhance community awareness and perform a host of other duties that will enhance the services for those with mental illness.
