RESOLUTION AUTHORIZING A SECOND AMENDMENT TO THE PROFESSIONAL AND TECHNICAL SERVICES MASTER CONTRACT WITH THE STATE OF MINNESOTA, ACTING THROUGH ITS DEPARTMENT OF EMPLOYMENT AND ECONOMIC DEVELOPMENT, TO PROVIDE VOCATIONAL REHABILITATION PRE-EMPLOYMENT TRANSITION SERVICES TO INCREASE THE CONTRACT AMOUNT BY $150,000 WITH THE TOTAL NOT TO EXCEED $450,000, TO EXTEND THE CONTRACT END DATE TO JUNE 30, 2022, AND TO MODIFY THE SERVICES AND FEES.

CITY PROPOSAL:
RESOLVED, that the proper city officials are hereby authorized to enter into Amendment No. 2 to the Vocational Rehabilitation Services Master P/T Contract, City Contract No. 24068, substantially in the form of the attached Exhibit A, with the state of Minnesota, department of employment and economic development (“DEED”), to increase the total compensation by $150,000, with the total not to exceed $450,000, payable to funds no. 268-031-6238-4220 (Workforce Development, Grants, ETS Fee for Service, State of MN), to extend the contract end date to June 30, 2022, and to modify the services and fees.

STATEMENT OF PURPOSE: The purpose of this resolution is to approve a contract amendment with the Minnesota department of employment and economic development to increase the total compensation by $150,000, with the total not to exceed $450,000, to extend the contract end date to June 30, 2022, and to modify the services and fees.
